New Substances Data Form
Items not purchased through the Central Processing Facility may be used, but first
entered into inventory by filling out this New Substances Data Form before being brought into the facility. All items not
on inventory will be removed. All gases are to be ordered through Central Processing Facility
accounting office by filling out a Cylinder Order Form. Please read the Gases & Chemical
